5-2-1-0 Healthy Choices Count! Community Grants

These communities in Iowa have received grant funding from the Iowa Department of Public Health to support 5-2-1-0 Healthy Choices Count!

The grants are a three-year tiered cycle from IDPH. Funding is provided to multi-sector community coalitions to make sustainable and equitable environmental and policy changes that support active living and healthy eating. This project is funded by the State of Iowa Health and Human Services Appropriations for Childhood Obesity Prevention.
