What has been your biggest lesson learned with incorporating 5-2-1-0 Heath Care to your practice?

My biggest lesson learned is that it’s important to (incorporate 5-2-1-0) at all Well-Child Visits and not just those visits where there is a clear concern about a child’s weight. Since incorporating the screening tool into all my Well-Child Visits over two months ago, I have really appreciated the importance of universal screening and consistent messaging for all patients and families.

Have you seen your patients positively impacted by 5-2-1-0? In what ways?

The positive messaging plus the ability for patients and their families to choose what they want to work on, if anything, is empowering. I have seen patients make great changes bit by bit when tackling what they are ready to do, when they are ready to do it.

Is there a piece of 5-2-1-0 Health Care that has changed the way you administer a Well-Child Visit?

After starting this screening tool, I have specifically reviewed all aspects of 5-2-1-0 in a more comprehensive manner including meals with families, sleep amount and frequency of breakfast. I think including these issues with the 5-2-1-0 questions shows families the importance of these issues too when it comes to healthy lifestyle habits.

In what ways do you see your facility growing in the 5-2-1-0 Health Care efforts?

I started the 5-2-1-0 questionnaire this spring in only my weight follow-up visits and incorporated it into all my Well-Child Visits starting at two years old this past August. Since the IMS team came to our office in September, we have rolled out the forms to all our pediatrics and family medicine staff for all Well-Child Visits. It’s been an addition that has been embraced by all staff.

How has your clinic/staff been impacted by incorporating 5-2-1-0?

Having a positive message to provide consistently to all families is really empowering not just to patients and their families but also to the staff that works with them. My nurses and medical students are all confident in giving good advice regarding healthy habits as it’s all based on the 5-2-1-0 messaging.
